How much does it cost to open a med spa in the US?
Most new med spas need somewhere between about $50,000 and well over $1 million in cash before the first patient walks in. A solo injector renting a treatment room sits at the low end. A two-room boutique med spa usually lands in the low-to-mid six figures. A multi-room clinic with lasers and body devices can pass $1 million. The calculator’s four starting models follow that spread, and every line is editable.
The total moves on four things: whether you build out your own space, which devices you buy on day one, whether you pay an injector or treat patients yourself, and how many months of costs you keep in reserve.
Costs US calculators often miss
- Medical director. In most states a physician (or, in some states, an NP) must oversee medical treatments. Monthly retainers commonly fall between $1,500 and $5,000, and more for on-site oversight. It’s a monthly cost from day one, not a launch cost.
- Good faith exams and chart review. Many states require a qualified provider to examine each patient before injectables or prescription treatments. Budget for it monthly if your medical director doesn’t cover it.
- Malpractice and liability cover. Professional liability for the injector, general liability and property cover for the space, and workers’ comp once you hire.
- The phone. New med spas lose bookings to unanswered calls before they lose them to competitors. A third of calls to healthcare businesses go unanswered, according to CallRail’s 2025 data. A missed-call text-back and a CRM cost far less than one lost patient a month.
What the starting models assume
| Model | Who it fits | Typical cash to open |
|---|---|---|
| Solo injector | NP or RN injector renting a room or suite, treating patients yourself, no devices | $40K–$70K |
| Boutique med spa | 1–2 treatment rooms, one entry-level device, front desk and an aesthetician | $180K–$300K |
| Full med spa | 3–4 rooms, laser, employed NP injector, larger launch budget | $400K–$600K |
| Premium flagship | Multiple providers, several devices, prime retail location | $1M+ |
How to read the runway check
Most new med spas lose money for the first few months while the schedule fills. The chart shows each month’s profit or loss based on your revenue ramp. If the deepest cash dip is bigger than your reserve, you either need more cash before opening or a faster ramp. The fastest ramp levers are rarely the expensive ones: a Google Business Profile that’s live and collecting reviews before opening day, online booking on every treatment page, and a text that goes out within a minute of every missed call.
Once you’re open, use our free free med spa marketing audit to score how well you turn calls and searches into bookings and get a 90-day plan.

Frequently asked questions
How much does it cost to open a med spa?
Roughly $50,000 for a lean solo injector setup up to $1 million or more for a multi-room clinic with devices. A typical one- to two-room med spa needs about $180,000 to $300,000, including a few months of operating costs in reserve. Use the calculator above with your own rent, payroll, and device plans for a closer number.
What is the biggest startup cost for a med spa?
Usually build-out and devices. A single laser or body-contouring device can cost more than the rest of your launch combined. Leasing spreads that cost but adds a monthly payment. Starting with injectables and skin treatments and adding devices once demand is proven keeps the opening cost down.
How much does a med spa medical director cost?
Monthly retainers commonly run from about $1,500 to $5,000, depending on your state, the treatments you offer, and how often the director is on site. Some directors charge per treatment or a share of medical revenue instead. Check your state’s supervision rules before signing.
How long does it take a med spa to become profitable?
It depends on your monthly costs and how fast your schedule fills. Many new med spas take several months to over a year. The runway check above shows your break-even month based on the visit numbers you enter, and how much cash you need to get there.
Can I open a med spa if I’m not a doctor?
In many states, yes, with a physician medical director and the right business structure. Some states restrict who can own a medical practice, which affects how med spas are set up. Speak to a healthcare attorney in your state before you form your company or sign a lease.
How much is malpractice insurance for a med spa?
Professional liability for a med spa is commonly quoted at around $2,000 to $5,000 a year per provider, and more for laser and higher-risk treatments or in states with higher claim costs. Add general liability and property cover for the space, and workers’ comp once you hire. Get quotes from carriers that specialize in aesthetics before you sign a lease.
What is a good profit margin for a med spa?
Margins vary widely with your service mix, payroll, and how full your schedule is. The useful number for planning is contribution margin per visit: what’s left after product and supplies. The runway check above uses it to show how many paid visits a month cover your fixed costs, which tells you more about profitability than an industry average.
How much should a new med spa spend on marketing?
Plan for a one-time launch budget (brand, website, photography, opening campaign) plus a monthly budget for local SEO, your Google Business Profile, and ads. Spend on getting found and on answering every inquiry fast before you spend on more ads. An unanswered call wastes the ad that produced it.
